In the summer of 2005 I asked new coaches and coaches in training what forms and templates would be useful to them in setting up their practice.
These downloads are as a result of that research.
They are all very basic forms and you will be able to adapt and improve them to suit your own ways of working. In order to read these files you should have Adobe Reader installed on your pc. If you don't have this software, you can download it by clicking on the logo and following the instructions on the web page you will be directed to.
